While the federal government's efforts to expand Medicaid has faced opposition from lawmakers and governors in conservative states, several states have expanded Medicaid coverage through the a process known as the "private option" approach. This approach expands Medicaid by selling private health insurance policies subsidized by the states. North Dakota is trying another approach that many see as an outlier to what some of the more conservative states may be willing to try. The state has invited private health insurance companies to bid for the business to cover the more-than 20,000 adults that are now eligible for Medicaid.

Nick Manetto, director at FaegreBD Consulting, was featured in Health Leaders Media on how the North Dakota model could eventually be a politically acceptable way for lawmakers in conservative states, who are typically staunchly against any facet of the Patient Protection and Affordable Care Act, to expand Medicaid. Despite the unique approach North Dakota is taking, Manetto expects a long road ahead for the states that have resisted expansion.

"The arguments against it are focused on the reputation of Medicaid today," Manetto said. "It will be a long slog to move Medicaid expansion in the states that remain."
